Сравнительная характеристика системных антимикотиков в терапии микозов кистей и стоп с онихомикозом (реферат, курсовая, диплом, контрольная)
В настоящее время отмечается значительный рост заболеваний микотической этиологии, что связано и с улучшением их диагностики, и с количественным увеличением факторов, способствующих их развитию. Онихомикозы относятся к наиболее часто встречающимся микотическим заболеваниям человека. По образному выражению Roberts D.T., «Онихомикозы являются заболеваниями развитого мира» [280]. Проблема адекватной терапии онихомикозов является достаточно значимой, учитывая их широкую распространенность: такие больные составляют 2−5% всего населения [166]. Значимость этой проблемы была замечена крупнейшими фармацевтическими фирмами, которыми были предложены многочисленные лекарственные антифунгальные препараты для лечения онихомикозов. К сожалению, многие публикации последних лет, посвященные современным антифунгальным препаратам, как в зарубежной, так и в отечественной литературе, носят преимущественно рекламный характер, в то время как проблема лечения онихомикозов не всегда решается эффективно.
Лечение больного с онихомикозами в ряде случаев представляет собой сложную задачу и требует применения не только традиционных наружных средств терапии, но и антифунгальных препаратов, обладающих системным действием, а также средств, коррегирующих состояние иммунной, нервной, сосудистой систем. К сожалению, в современной литературе недостаточно ясно сформулированы обоснования выбора адекватного способа лечения онихомикоза: отдавать ли предпочтение традиционным, наружным методам лечения (в частности, удалению ногтевых пластин хирургическим путем), либо выбирать системные антифунгальные препараты, с учетом противопоказаний к их использованию.
На сегодняшний день не разработан также общеклинический минимум исследований, необходимых для решения вопроса о возможности назначения противогрибкового препарата системного действия.
В ряде иностранных публикаций прослеживается динамика изменений гемограммы и некоторых биохимических показателей сыворотки крови в процессе лечения системными антимикотикамик сожалению, в отечественной литературе подобных публикаций обнаружить не удалось. Отсутствуют также данные о динамике изменений иммунного статуса в процессе лечения антифунгальными препаратами общего действия и по окончании терапии.
Цель и задачи исследования
.
Целью исследования явилась разработка оптимальных схем этиотропной и патогенетически обоснованной терапии микозов кистей и стоп с онихомикозом с использованием системных антифунгальных препаратов (тербинафин, итраконазол, гризеофульвин).
Для достижения поставленной цели в ходе работы решались следующие задачи:
1. Сравнение динамики изменений клинической картины и результатов микроскопического и культурального микологического исследования у больных, получавших различные системные антифунгальные средства.
2. Изучение и сравнение безопасности новых антифунгальных препаратов — итраконазола и тербинафина, а также побочных явлений, развивающихся в процессе терапии данными средствами.
3. Изучение и сравнение характера изменений результатов общеклинических исследований (клинический анализ крови, общий анализ мочи), биохимических показателей сыворотки крови (билирубин, AJIT, ACT), данных сонографического исследования гепатопанкреатической зоны у пациентов с микозами кистей и стоп с онихомикозом в процессе терапии различными системными антимикотическими препаратами и по окончании терапии ими.
4. Сопоставление показателей клеточного иммунитета в процессе терапии и по окончании лечения системными антимикотиками.
5. Оптимизация подбора системного антифунгального средства путем определения индивидуальной антимикотикограммы с целью усовершенствования методов лечения пациентов с микозом кистей и стоп с онихомикозом.
Научная новизна исследования.
1. Впервые путем сравнения результатов лечения онихомикоза системными антимикотиками — тербинафином и итраконазолом — показаны возможности безопасного применения данных препаратов с высокой степенью эффективности.
2. Впервые на основании сопоставления данных исследования состояния печени, системы крови, иммунной системы в процессе лечения различными антифунгальными препаратами общего действия сформулированы клинические основания для выбора антимикотика в зависимости от этиологии, особенностей клинического течения онихомикоза и фоновой патологии у данной группы больных.
3. Предложен необходимый минимум исследования перед назначением терапии антифунгальным препаратом общего действия и определен перечень клинико-лабораторных данных, используемых при мониторировании в ходе лечения системным антимикотиком с учетом фоновой патологии у конкретного больного.
4. Впервые показана целесообразность использования индивидуальной антимикотикограммы с целью рационального выбора антифунгального препарата общего действия.
Практическая значимость работы.
Определен перечень необходимых клинико-лабораторных исследований перед назначением терапии антифунгальным препаратом общего действия и в процессе лечения данным лекарственным средством. Внедрение необходимого минимума исследования в практическую работу позволяет максимально предупреждать развитие возможных неблагоприятных побочных реакций в процессе лечения системным антимикотиком.
Использование в процессе выбора того или иного антифунгального препарата системного действия индивидуальной антимикотикограммы позволяет наиболее рационально подобрать системный антимикотик.
Основные положения, выносимые на защиту.
1. Онихомикозы возникают и существуют, как правило, на фоне сопутствующих заболеваний (преимущественно — желудочно-кишечного тракта и печени, сосудистых и эндокринных заболеваний).
2. Тербинафин и итраконазол относятся к системным антифунгальным препаратам, позволяющим достичь высокой клинико-лабораторной излеченности онихомикозов.
3. Сочетание высокой лечебной эффективности с безопасностью тербинафина и итраконазола позволяют без вреда для больного получать положительный результат у преобладающего большинства больных. Лишь у отдельных больных функциональные нарушения органов или явления индивидуальной непереносимости служат основанием для отмены препаратов.
4. Способом достижения максимальной безопасности антифунгальных препаратов общего действия является мониторирование в процессе терапии ими показателей уровня лейкоцитов периферической крови, активности аминотрансфераз и уровня билирубина сыворотки крови.
Апробация и реализация результатов исследования.
Основные положения работы доложены на III и IV Российском национальном конгрессе «Человек и лекарство» (1996, 1997 гг.), на городской научно-практической конференции дерматологов, акушер-гинекологов и урологов Санкт-Петербурга в 1998 году, на I конгрессе дерматовенерологов.
России в 2003 году, III, IV, V, VI и VII Кашкинских чтениях (2001, 2002, 2003, 2004 г. г.).
По теме диссертации опубликована 21 научная работа.
Результаты исследования нашли применение в практической работе клиники и консультативно-диагностического отделения НИИ медицинской микологии СПб МАЛО, Гатчинской клинической больницы и кожно-венерологического отделения Гатчинской поликлиники, медицинского центра ООО «Медэкспресс». Материалы работы используются в педагогическом процессе кафедры клинической микологии, аллергологии и иммунологии, кафедры кожных и венерических болезней СПб МАЛО.
Выводы.
1. У большинства больных онихомикозами стоп и кистей обнаруживаются хронические заболевания органов пищеварения, печени, эндокринных желез и других органов. Это позволяет относить онихомикозы к вторичным заболеваниям, возникающим на фоне нарушения обменных и защитных функций, вызванных общесоматическими хроническими заболеваниями.
2. Применение современных антифунгальных препаратов — тербинафина и итраконазола — позволяет получить принципиально новый стойкий положительный результат (микологическое и клиническое излечение) у подавляющего большинства больных.
3. Использование индивидуальной антимикотикограммы способствует оптимальному подбору антимикотического средства для каждого больного.
4. Безопасность лечения онихомикозов тербинафином и итраконазолом определяется их селективным влиянием на ферментативные системы грибковой клетки, отсутствием действия на аналогичные структуры человека, что доказывается проведением лабораторно-инструментального контроля состояния органов и систем.
5. Сочетание высокой лечебной эффективности тербинафина и итраконазола с безопасностью для больных, возможность мониторного наблюдения за состоянием органов и систем практически возможно в условиях лечебно-профилактических учреждений всех уровней, что позволяет постоянно расширять показания к их назначению и совершенствовать методы их применения в соответствии с индивидуальными клиническими особенностями заболевания и состоянием больного.
6. У большинства больных побочных влияний системных антифунгальных препаратов на показатели работы печени и состав периферической крови не выявлено. У отдельных больных отмечается повышение активности аминотрансфераз и снижение уровня лейкоцитов периферической крови.
7. При сравнении гризеофульвина, итраконазола и тербинафина по воздействию на состав периферической крови, активность аминотрансфераз и уровень билирубина сыворотки крови статистически достоверных отличий не установлено.
Практические рекомендации.
1. Перед назначением антифунгального препарата общего действия рекомендуется клиническое исследование крови (уровень лейкоцитов) и биохимический анализ сыворотки крови (уровень билирубина, активность аланинаминотрансферазы, аспартатаминотрансферазы).
2. В процессе терапии системным антимикотиком необходимо мониторное лабораторное наблюдение за больным. Это позволяет предупреждать развитие неблагоприятных побочных реакций путем регулирования дозы и назначения органопротективных средств.
3. Для оптимизации подбора системного антифунгального препарата рекомендуется определение индивидуальной антимикотикограммы.
