В настоящее время с лечением туберкулеза во всем мире сложилась столь неблагополучная ситуация, что Всемирная Организация Здравоохранения объявила в 2003 г туберкулез глобальной угрозой жизни человека. Невысокая эффективность лечения туберкулеза обусловлена многими причинами, в том числе токсичностью многих противотуберкулезных препаратов (ПТП). Особенно выражены токсические эффекты у препарата первого ряда — рифампицина, которые вынуждают больных отказываться от его приема.
Для решения проблемы лечения туберкулеза необходимы новые подходы, которые должны включать как поиск новых препаратов, так и разработку новых менее токсичных лекарственных форм ранее применяемых ПТП.
В связи с тем, что микобактерия туберкулеза (Mycobacterium tuberculosis (М.tuberculosis)), вызывающая это заболевание, размножается в основном внутри клеток легочной ткани (макрофагов), представлялось целесообразным использовать такие лекарственные формы, которые были бы способны направленно транспортировать ПТП внутрь этих клеток. Эта задача может быть решена с помощью липосом, которые, как известно, фагоцитируются макрофагами.
Наряду с этим представляется необходимым поиск новых препаратов, обладающих антибактериальной активностью, но нетоксичных для человека. В качестве примера можно привести белки, секретируемые непатогенными бактериями (бактериоцины). Однако эти водорастворимые белки не проникают через мембрану макрофагов и для их внутриклеточной доставки можно использовать липосомы.
Одним из положительных свойств липосом в качестве транспортирующей системы является фармакологическая активность самих фосфолипидов, из которых сформированы липосомы. Известны несколько препаратов на основе фосфолипидов для лечения гепатита, цирроза, токсических поражений печени: «Эссенциале» (Германия), отечественный препарат «Фосфоглиф», препарат «Липин» (Украина) и др. В литературе имеются отдельные сообщения о противовоспалительном и антиэксудативном действии фосфолипидов. Перечисленные выше данные позволяют прогнозировать значительные преимущества липосомальных форм ПТП по сравнению с таблетированными, в связи с чем разработка таких форм представляется весьма актуальной. Она позволит увеличить растворимость ПТП, усилить их проникновение в макрофаги и возможно, окажет дополнительные эффекты, например, снизит токсичность или увеличит время пребывания препарата в организме.
Наряду с этим следует учесть, что в легких человека и животных липосомы, содержащие ПТП, целенаправленно взаимодействуют как с инфицированными макрофагами, так и с микобактериями. Известно, что клеточная стенка последних содержит различные фосфолипазы, в том числе фосфолипазу С и фосфолипазу А2. В связи с этим с целью прогнозирования возможных механизмов деградации компонентов липосом при их введении в организм представлялось необходимым оценить свойства липосом после их обработки различными фосфолипазами.
Основной целью работы являлась разработка научных основ создания липосомальных форм лекарственных препаратов и биологически активных соединений как способа оптимизации их свойств для повышения эффективности фармакологического действия. Другой целью была оценка антибактериальной активности липосомальной формы рифампицина в отношении клеток М. tuberculosis in vitro и in vivo.
Для достижения поставленных целей необходимо было решить следующие задачи, которые разделялись на три группы:
1.1. Изучить закономерности гидролиза фосфолипидов в различных агрегационных состояниях фосфолипазой С.
1.2. Исследовать слияние и проницаемость для ионов кальция липосом из смесей фосфолипидов при инкубации их с фосфолипазами С разной специфичности.
1.3. Те же процессы изучить на изолированных нервных окончаниях (синаптосомах).
1.4. Оценить эффективность действия липосом, содержащих продукт гидролиза фосфолипидов — 1,2-диацилглицерин (ДАГ) — на функциональное состояние фагоцитов человека и рану кожи экспериментальных животных.
2.1. Получить комплексы белковых ингибиторов трипсина (основного панкреатического ингибитора протеаз и соевого ингибитора типа БауманаБирк) и оценить их ингибирующую активность в составе комплексов.
2.2. Получить белок-липидные комплексы с бактериоцинами и исследовать их антимикробную активность in vitro и in vivo в отношении m.tuber.
3.1. Изучить физико-химических свойства антибиотиков рифамицинового ряда в зависимости от рн средыоптимизировать условия получения их липосомальных форм.
3.2. Исследовать стабильность липосом с рифампицином при хранении.
3.3. Исследовать антибактериальное действие липосом с рифампицином in vitro и in vivo в отношении М.tuberculosis.
выводы.
1. Обнаружено, что физико-химические свойства модельных мембран, изученные на примере однослойных везикул, изменяются при введении в их состав метаболита фосфолипидов 1.2-диацилглицерина: уменьшается стабильность липосом и их мембраны становятся проницаемыми для ионов кальция.
2. Показано, что механизм этого явления обусловлен способностью 1.2-диацилглицерина модифицировать бислойную упаковку фосфолипидов.
3. Для биологических мембран установлено, что только 1.2-диацилглицерин, образовавшийся под действием специфичной фосфолипазы С, способен формировать кальциевые каналы проницаемости, которые ингибируются верапамилом. Аналогично действует арахидоновая кислота. Неспецифичная фосфолипаза С и другие исследованные жирные кислоты такого эффекта не оказывают.
4. Обнаружено, что введение 1.2-диацилглицерина и арахидоновой кислоты в однослойные везикулы усиливает их действие на клетки и ткани: возрастает интенсивность кислородного взрыва макрофаговувеличивается скорость заживления операционной раны кожи.
5. Показано образование гидрофобных комплексов водорастворимых белков-ингибиторов протеаз (основного панкреатического ингибитора трипсина и соевого ингибитора Баумана Бирк) с мультиламеллярными везикулами не только из отрицательно заряженных фосфолипидов, но и из цвиттер-ионов.
6. Установлено сохранение ингибирующей активности белков в составе комплексов, которая увеличивается в присутствии ионного детергента из-за освобождения активного центра ингибиторов, ранее блокированного в составе комплекса.
7. Получена липосомальная форма бактериоцина, которая подавляет рост М. tuberculosis H37Rv не только в культуральной среде, но и в инфицированных макрофагах. Лечение этой формой бактериоцина животных, зараженных летальной дозой М. tuberculosis H37Rv, достоверно увеличивает продолжительность жизни.
8. Установлено, что при физиологических условиях рифампицин и рифабутин существует в различных ионизационных формах и по-разному связываются с однослойными везикулами, содержащими отрицательно заряженные фосфолипиды.
9. Включение противотуберкулезных антибиотиков в липосомы при определенных условиях позволяет увеличить содержание в водной фазе рифампицинав 10 раз, а рифамбутинав 100 раз.
10. При изучении стабильности липосомальной формы рифампицина в виде раствора и лиофилизированного порошка при хранении обнаружено, что оба препарата сохраняют высокую антибактериальную активность в отношении М. tuberculosis H37Rv, но только один из них (лиофилизированный порошок) выдерживает хранение в течение 1 года.
11. В опытах на животных, зараженных М. tuberculosis H37Rv, установлено, что разработанная нами липосомальная форма рифампицина обладает повышенной эффективностью действия по сравнению с водным раствором антибиотика и при этом не является гепатотоксичной.
ЗАКЛЮЧЕНИЕ
.
В завершение следует сказать, что в результате проведенных нами исследований были выявлены различия в действии на биологические мембраны фосфолипаз С различной специфичности. Это позволило предложить механизм нарушения слияния фагосом с лизосомами в макрофагах, инфицированных микобактериями.
Главные из полученных результатов относятся к двум взаимосвязанным направлениям:
1. фундаментальному, в котором рассмотрена динамика взаимодействия сложной системы: микобактерии — макрофаги — лекарственные соединения с учетом того, что микобактерии, расположенные в фагосомах макрофагов, модифицируют фосфолипидный состав фагосомальных мембран. В частности обнаружено, что ДАГ, образованный под действием бактериальной фосфолипазы С, вызывает слияние мембран, но не влияет на уровень ионов кальция. Специфичная к ФИ фосфолипаза С, которая присутствует в фагосомальной мембране, способствует как слиянию мембран, так и повышению содержания ионов кальция, без чего невозможно дальнейшее созревание фагосом.
Другой важный результат этих исследований состоит в том, что впервые обнаружено, что многие фосфолипиды вызывают изменение чувствительности микобактерий к различным противотуберкулезным препаратам, в том числе к антибиотикам, и раскрыт механизм этого явления.
2. прикладному, которое состояло в разработке новых лекарственных форм антибиотиков, обладающих активностью в отношении М.tuberculosis. Изучение широкого набора физико-химических характеристик рифампицина и рифабутина позволило не только объяснить закономерности их антимикробной активности, но и разработать липосомальные формы препаратов, в которых в несколько раз (а в случае рифабутинав десятки раз) увеличено содержание антибиотиков в водной фазе.
В ходе изучения стабильности при хранении различных форм липосомального рифампицина установлено, что длительное хранение выдерживает только лиофилизированный порошок. Это позволит в дальнейшем перейти к практическому использованию данного препарата.
