Антигенная изменчивость дериватов полиовирусной вакцины (реферат, курсовая, диплом, контрольная)
Полиовирус является этиологическим агентом полиомиелита — острого заболевания центральной нервной системы. Со времени создания эффективных вакцин, сначала инактивированной, а затем живой аттенуированной, паралитический полиомиелит перестал быть неконтролируемым заболеванием. На сегодняшний день циркуляция полиовирусов дикого типа блокирована в большинстве стран мира (Kew et al., 2005).
Оральная полиовирусная вакцина (ОПВ), включающая в себя живые аттенуированные штаммы трех серотипов полиовируса (Sabin and Boulger, 1973), является одной из наиболее эффективных и безопасных. Именно она в основном используется для иммунизации людей против полиомиелита. До недавнего времени единственным негативным следствием применения ОПВ считалось развитие в очень редких случаях вакцинно-ассоциированного полиомиелита (ВАПП) у реципиента или непосредственно контактирующих с ним лиц. Это явление связано со способностью вакцинных штаммов в короткие сроки восстанавливать нейровирулентные свойства при репродукции в человеческом организме. Несмотря на эту особенность, предполагалось, что производные ОПВ не способны к длительной трансмиссии, в отличие от полиовирусов дикого типа, и поэтому не несут эпидемической опасности (Hull and Aylward, 1997; Woodetal., 2000).
Однако, недавние исследования показали, что некоторые вакцинно-родственные вирусы способны к продолжительной циркуляции, и в некоторых случаях могут вызывать вспышки паралитического полиомиелита (Kew et al., 2005). На сегодняшний день становится ясно, что трансформация вакцинных штаммов в подобные производные (обозначаемые как VDPV, от англ. vaccine-derived polioviruses) является не исключением из общего правила, а скорее закономерностью их природной эволюции.
Наряду с восстановлением нейровирулентных свойств, еще одним общеизвестным следствием природной репродукции вакцинных штаммов является изменение их антигенных характеристик (Crainic et al" 1983; Minor et al., 1986b). Эта особенность широко используется при проведении внутритиповой дифференциации (ВТД) полиовирусов с применением различных панелей моноклональных антител.
Crainic et a!., 1983) или перекрестно-адсорбированных поликлональных сывороток (van Wezel and Hazendonk, 1979; van der Avoort et al, 1995). Более того, по указаниям Всемирной Организации Здравоохранения (ВОЗ), для полиовирусов, проявляющих в ВТД измененные антигенные свойства по сравнению с вакцинными штаммами, должно проводиться дальнейшее детальное исследование, направленное на выявление VDPV (ВОЗ, 2005).
Несмотря на большое количество накопившихся сведений об изменчивости антигенных свойств вакцинно-родственных штаммов, вопрос о том, какова ее роль в природной эволюции полиовируса, широко не обсуждался.
Проведенное в настоящей работе исследование способствует более глубокому пониманию эволюционных механизмов и следствий антигенной изменчивости вакцинно-родственных штаммов.
Цель и задачи исследования
.
Целью настоящей работы явилось исследование природной антигенной изменчивости ОПВ-дериватов.
В задачи работы входило: идентификация мутаций в антигенных сайтах, ответственных за изменение антигенных свойств вакцинно-родственных штаммовисследование профилей реактивности с моноклональными антителами ОПВ-дериватов, имеющих мутации в отдельных антигенных сайтахоценка способности дериватов полиовирусной вакцины уходить из-под иммунного ответа.
Научная новизна и практическая ценность работы.
Проведенное в настоящей работе исследование способствует пониманию роли антигенной изменчивости в природной эволюции полиовируса, ее биологического смысла.
В работе впервые детально сопоставлены антигенные характеристики и конкретные изменения в антигенных эпитопах ОПВ-дериватов. Показано, что антигенные изменения возникают уже на самых ранних этапах природной эволюции вакцинно-родственных штаммов. Продемонстрировано, что фактором фиксации мутаций в антигенных эпитопах является не приобретение ОПВ-дериватами способности уходить от иммунного ответа, а, вероятнее, оптимизация других важных этапов жизненного цикла, таких как взаимодействие с рецептором или сборка вирионов. Выдвинута гипотеза, что антигенные изменения ведут к быстрому и закономерному восстановлению структуры капсида ОПВ-дериватов, характерной для полиовирусов дикого типа. Впервые систематически регистрируемые в антигенных детерминантах мутации рассматриваются как класс де-аттенуирующих генетических изменений, повышающих жизнеспособность ОПВ-дериватов в естественных условиях репродукции. Полученные данные позволяют заключить, что антигенная изменчивость является характерным свойством вакцинно-родственных штаммов, представляет собой начальный этап их природной эволюции и является предпосылкой для возникновения высоконейровирулентных VDPV, способных циркулировать и вызывать паралитический полиомиелит. Подобные высокопатогенные агенты имеют большое эпидемиологическое значение в свете Программы Всемирной Организации Здравоохранения (ВОЗ) по глобальной ликвидации полиомиелита. В настоящей работе показано, что отдельные мутации в антигенных эпитопах ОПВ-дериватов приводят к формированию специфического профиля реактивности с панелями моноклональных антител. Составление антигенных профилей представляет собой новый удобный метод четкой детекции антигенных изменений у полевых изолятов полиовируса. Таким образом, этот анализ может быть использован для ВТД полиовирусов, обнаруживаемых в ходе эпидемиологического надзора, направленного на выявление VDPV.
2. ОБЗОР ЛИТЕРАТУРЫ.
выводы Показана ранняя и систематическая фиксация мутаций в определенных аминокислотных позициях капсидных белков ОПВ-дериватов. Мутации, как правило, наблюдаются в районах антигенных детерминант и в большинстве своем затрагивают остатки, уникальные для вакцинных штаммов по сравнению с вирусами дикого типа. Выдвинута гипотеза, что подобная изменчивость направлена на исправление генетических дефектов, приобретенных вакцинными штаммами в процессе искусственного отбора или в ходе природной эволюции их предшественников. Таким образом, наиболее распространенные среди независимых ОПВ-дериватов антигенные изменения могут быть причислены к классу де-аттенуирующих. Показано, что человеческие сыворотки эффективно нейтрализуют вакцинно-родственные штаммы, несущие мутации в антигенных эпитопах. Следовательно, приобретение способности уходить от иммунного ответа не является главным фактором фиксации антигенных изменений у ОПВ-дериватов. Фиксация этих изменений может быть обусловлена оптимизацией различных стадий репродукции полиовируса, таких как взаимодействие с рецептором, высвобождение РНК, сборка вирусной частицы. Проведено сравнение двух методов, используемых для характеристики антигенных свойств полиовирусов. Показано, что широко используемый ИФА с поликлональными антителами способен детектировать изменения лишь немногих аминокислотных остатков в антигенной области вируса (единственного остатка для серотипа 1 и одного/двух — для серотипов 2 и 3). Напротив, недавно разработанный метод получения профилей реактивности ОПВ-дериватов с моноклональными антителами выявляет существенно больший спектр мутаций антигенной области. Таким образом, последний подход более информативен и может быть рекомендован для эпидемиологического надзора за полиовирусами.

ЗАКЛЮЧЕНИЕ
.
Присутствие мутаций в отдельных антигенных эпитопах является характерной особенностью вакцинно-родственных штаммов полиовируса. В настоящей работе были выявлены закономерности фиксации этих антигенных изменений, а также сделаны предположения об их роли в природной эволюции ОПВ-дериватов. Регулярная фиксация некоторых мутаций в АгО (как правило, приводящих к элиминации вакциноспецифичных остатков уже на ранних этапах эволюции ОПВ-дериватов) свидетельствует, что эти изменения могут оказывать благоприятное влияние на жизнеспособность вируса и принадлежат к классу де-аттенуирующих мутаций. Антигенная изменчивость вакцинно-родственных полиовируеов, по-видимому, мало способствует их уходу от иммунного ответа. Однако, она свидетельствует об общей способности вакцинных штаммов быстро адаптироваться к естественным условиям размножения и приобретать свойства, присущие полиовирусам дикого типа. Биологический смысл этих изменений, вероятно, состоит в восстановлении благоприятной структуры капсида, поврежденной в процессе исскуственной селекции вакцинных штаммов или в ходе природной эволюции их предшественников. Возвращение характерной для полиовируеов дикого типа конформации поверхности вириона может восстанавливать благоприятные условия для проникновения вируса в клетку, сборки вирионов или других этапов его репродукции.
Вероятность трансформации вакцинного штамма в высоконейровирулентный VDPV, способный циркулировать и вызывать полиомиелит у восприимчивых людей, является одной из самых важных эпидемиологических проблем. Своевременное выявление в ходе эпидемиологического надзора таких производных ОПВ позволит правильно планировать противоэпидемические мероприятия. В настоящей работе представлен новый, более информативный, чем стандартная ВТД, метод характеристики антигенных изменений ОПВ-дериватов, который может быть использован для скрининга полевых изолятов полиовируса.
