Данные эпидемиологических исследований показали, что частота инсультов и большинства клинических проявлений ишемической болезни сердца (ИБС), в том числе инфаркта миокарда, внезапной смерти и сердечной недостаточности, выше у пациентов с артериальной гипертензией (АГ) (Шальнова С.А. и соавт., 2002; Flack J.M. et al., 1995; JNC-VII, 2003). Гипертоническая болезнь (ГБ) принадлежит к наиболее распространенным и, учитывая связь с высоким риском смертности от сердечно-сосудистых осложнений, социально значимым заболеваниям (Оганов Р.Г., 2002; Бритов А. Н., 2003).
Гипертрофия левого желудочка (ГЛЖ), выявляемая при эхокардиографическом исследовании почти у трети больных ГБ, является самостоятельным независимым фактором риска сердечно-сосудистых заболеваний (Levy D. et al., 1990; Devereux R.B., Roman M.J., 1995; Lip G.Y.H. et al., 2000; Verdecchia P. et al., 2001). Важное прогностическое значение ГЛЖ определяет интерес к изучению патогенеза гипертрофического процесса, оценки перспектив его обратного развития и поиску медикаментозных средств для его коррекции. Гипертрофия миокарда при ГБ сопровождается увеличением сердечной стромы. Поэтому термин «ремоделирование» наиболее полно характеризует процесс структурных изменений в миокарде в условиях нагрузки давлением. Основное внимание при изучении ремоделирования миокарда у больных ГБ уделено роли гемодинамических факторов (Liu J.E. et al. et al., 1999; Mancia G., Parati G., 2000). В последние годы активно обсуждается участие симпатоадреналовой и ренин-ангиотензин-альдостероновой систем в развитии гипертрофии миокарда (Cooper G., 1997; Swynghedauw В., 1999).
Миокард представляет собой сложную интеграцию клеток различного происхождения, связанных между собой системой межклеточного взаимодействия и ауто/паракринной регуляции. На клеточном уровне морфологические изменения в миокарде реализуются с помощью трех основных процессов: клеточного роста, апоптоза, а также ассоциированного с ними интерстициального фиброза (Hefti М.А. et al, 1997; Weber КТ., 2001; Nadal-Ginard В. et al., 2003). Нарушение баланса между индукторами и ингибиторами клеточного роста, апоптоза и обмена экстрацеллюлярного матрикса приводит к структурным изменениям в миокарде. Однако роль факторов межклеточного взаимодействия в ремоделировании миокарда у больных ГБ, а также их связь с гемодинамическими влияниями мало изучена.
В патогенезе гипертрофии миокарда важное значение имеют структурные изменения сосудов микроциркуляторного русла (Intengan H.D., Schiffrin E.L., 2001; Struijker Boudier H.A.J., 2002). Они определяют разнообразие типов поражения органов-мишеней и влияют на развитие заболевания. Но спорным остается вопрос: опережают ли структурные изменения сосудистой стенки формирование ГЛЖ, или они развиваются параллельно в рамках единого патофизиологического процесса, подчиняясь общим законам ремоделирования сердечно-сосудистой системы при ГБ? Гиперплазия средней оболочки сосудов с повышением ее жесткости за счет развития интерстициального фиброзахарактерные морфологические изменения сосудистой стенки, свойственные ГБ. Однако роль факторов межклеточного взаимодействия в патогенезе структурных изменений крупных и мелких артерий нуждается в дальнейшем изучении для подбора адекватных методов их медикаментозной коррекции. Исследования крупных артерий у больных ГБ проводились или на основе аутопсийного материала, или с помощью неинвазивных ультразвуковых методов. При этом отсутствуют данные сравнительного анализа морфологических изменений крупных артерий с результатами ультразвукового исследования сосудов. Изучение особенностей ремоделирования крупных артерий различных сосудистых регионов при ГБ актуально еще и с точки зрения патогенеза воспалительных изменений в сосудистой стенке.
Нарушение функционального состояния эндотелия играет важную роль в структурных изменениях миокарда и сосудов при ГБ (Vanhoutte P.M., 1989;
Vanhoutte P.M., Boulanger C.M., 1995; Bonetti P.O. et al., 2003). Эндотелиальные клетки, благодаря пограничному положению и продукции многочисленных паракринных факторов, принимают активное участие в регуляции сосудистого тонуса, сосудистой проницаемости, процессов воспаления и в изменении реологических свойств крови (Cines D.B. et al., 1998; Becker B.F. et al., 2000). Большинство сердечно-сосудистых осложнений, возникающих у больных ГБ, связаны с развитием артериальных тромбозов. Формирование протромботических условий при ГБ обусловлено нарушением реологических параметров крови и антиадгезионных свойств эндотелия (Ajmani R.S., 1997; Lip G.Y.H., 2000). Однако, характер причинно-следственных отношений функционального состояния эндотелия, реологических свойств крови и гемодинамических факторов при ГБ не определен. Открытым остается вопрос о роли нарушений реологических свойств крови в развитии структурных изменений сердца и сосудов.
АГ значительно увеличивает риск атеросклеротического поражения сосудистой стенки (Alexander R.W., Dzau V.J., 2000). Вместе с тем, патогенетические механизмы, объединяющие эти два патологических процесса, нуждаются в уточнении. Рассматривая атеросклероз с позиции иммуновоспалительного процесса, можно предположить, что причиной ускорения атерогенеза при ГБ является активация воспалительных реакций (Ross R., 1999). Следует отметить, что клинические исследования, подтверждающие присутствие воспалительных изменений при ГБ, немногочисленны и противоречивы. Отсутствуют данные о функциональном состоянии лейкоцитов у больных ГБ, их роли в развитии эндотелиальной дисфункции, а также участии в ремоделировании миокарда и сосудов.
Результаты последнего мета-анализа подтвердили отсутствие принципиальных различий между гипотензивными препаратами в снижении риска развития ИБС и инсульта, если достигается целевой уровень артериального давления (АД) (Staessen J.A. et al., 2003). Однако частота развития сердечной недостаточности не зависела от уровня снижения АД, и в этом случае выбор лекарственного препарата имеет принципиальное значение. Аналогичные результаты получены при сравнительном исследовании влияния антигипертензивной терапии на регресс ГЛЖ и структурных изменений сосудистой стенки у больных ГБ (Liebson P.R. et al., 1995; Schmieder R.E. et al., 1996; Teo K.K. et al., 2000; Lonn E.M. et al., 2001; Devereux R.B. et al., 2001). Эти данные свидетельствуют о неоднозначном влиянии лекарственных препаратов на факторы, участвующие в ремоделировании миокарда и сосудов, а также о значимости негемодинамических факторов в повреждении органов-мишеней при ГБ. Спорным остаётся вопрос регресса фиброзных изменений в миокарде на фоне обратного развития ГЛЖ. Поэтому влияние на процессы обмена соединительной ткани может стать важным условием при выборе антигипертензивной терапии у больных ГБ.
ЦЕЛЬ ИССЛЕДОВАНИЯ.
Изучить молекулярные и клеточные механизмы, лежащие в основе структурных изменений миокарда и сосудов при гипертонической болезни, разработать подходы к медикаментозной коррекции выявленных нарушений.
ЗАДАЧИ ИССЛЕДОВАНИЯ.
1. Оценить вклад гемодинамических, нейрогуморальных и ауто/паракринных факторов в ремоделирование миокарда при гипертонической болезни.
2. Охарактеризовать ремоделирование крупных и мелких артерий, роль гемодинамических и нейрогуморальных факторов в структурных изменениях сосудов при гипертонической болезни. Провести сравнительный клинико-морфологический анализ структурных изменений артерий мышечно-эластического и мышечного типов, оценить возможности медикаментозной коррекции выявленных изменений.
3. Изучить связь эндотелиальной дисфункции со структурными изменениями сосудов при гипертонической болезниуточнить роль оксида азота и его метаболитов в регуляции сосудистого тонуса при гипертонической болезни.
4. Оценить вклад изменений реологических свойств крови и функциональной активности лейкоцитов в ремоделирование миокарда и сосудов у больных гипертонической болезнью.
5. Провести сравнительную оценку действия основных групп антигипертензивных препаратов (ингибиторов ангиотензин-превращающего фермента, бета-адреноблокаторов, блокаторов кальциевых каналов) на функциональную активность клеток крови и гуморальные факторы, участвующие в ремоделировании миокарда и сосудов при гипертонической болезни.
Основные положения, выносимые на защиту.
1. В развитии гипертрофии миокарда при гипертонической болезни, наряду с гемодинамическими и нейроэндокринными факторами, большое значение имеют факторы ауто/паракринной регуляции, контролирующие процесс гипертрофии кардиомиоцитов и увеличение стромального компонента миокарда.
2. Процесс ремоделирования сосудов при гипертонической болезни носит системный характер и тесно связан с развитием эндотелиальной дисфункции.
3. Изменение функциональной активности клеток крови у больных гипертонической болезнью является дополнительным фактором ремоделирования миокарда и сосудов, действие которого опосредуется через повреждение эндотелия, развитие протромботических условий и воспалительных процессов в сосудистой стенке.
4. Обратное развитие структурных изменений миокарда и сосудов зависит не только от степени снижения артериального давления, но и от влияния антигипертензивной терапии на негемодинамические факторы ремоделирования.
Научная новизна.
В работе получены новые данные, доказывающие патогенетическую связь процессов гипертрофии кардиомиоцитов, клеточной гибели и сердечного фиброза при гипертонической болезни. Впервые предложен новый неинвазивный тест определения белка теплового шока 70кДа в сыворотке крови для диагностики повреждения органов-мишеней при гипертонической болезни. Показана роль трансформирующего ростового фактора-Pi в развитии гипертрофии кардиомиоцитов, увеличении стромального компонента миокарда, а также в структурных изменениях сосудистой стенки при гипертонической болезни.
Впервые в исследованиях in vitro показано, что ингибиторы гидрокси-метил-глютарил коэнзим, А редуктазы в терапевтических концентрациях подавляют процессы пролиферации сердечных фибробластов, их фенотипическую трансформацию и предупреждают развитие гипертрофии кардиомиоцитов.
Установлено, что в основе нарушения эндотелийзависимой вазодилатации в пробе с реактивной гиперемией лежат морфологические изменения в интиме лучевой артерии. Основным фактором, определяющим развитие интимальной гиперплазии, является степень артериальной гипертензии. Выявлены морфофукциональные особенности крупных артерий различных сосудистых регионов. Показано, что парадоксальные вазоконстрикторные реакции могут быть связаны с нарушением метаболизма оксида азота в условиях оксидативного стресса.
Выявлено повышение функциональной активности лейкоцитов и концентрации адгезионных молекул ICAM-1 в плазме крови у больных гипертонической болезнью, что отражает развитие провоспалительных условий и способствует дополнительному повреждению эндотелия.
Получены новые данные о повышении содержания сосудистого ростового фактора в плазме крови у больных гипертонической болезнью, а также его связи с развитием гипертрофии миокарда.
Практическая значимость работы.
Установлено, что при выборе антигипертензивной терапии у больных гипертонической болезнью необходимо учитывать эффект лекарственных препаратов на реологические свойства крови, функциональную активность лейкоцитов и эндотелия, являющихся важным фактором ремоделирования миокарда и сосудов.
Показано, что увеличение концентрации белка теплового шока 70кДа в плазме крови может служить неинвазивным маркером повреждения кардиомиоцитов при гипертрофии миокарда, а повышение содержания трансформирующего ростового фактора-Pi и N-терминального пептида проколлагена III в сыворотке крови отражением развития сердечного фиброза.
Выявлено, что лучевая артерия имеет более выраженные признаки интимальной гиперплазии, чем внутренняя грудная артерия, что может быть основой развития вазоспазма и в дальнейшем стать причиной формирования стенозов при использовании её в качестве шунта для реваскуляризации миокарда.

По результатам диссертации опубликовано 36 печатных работ.
ВЫВОДЫ.
1. Развитие гипертрофии левого желудочка у больных гипертонической болезнью зависит от степени гемодинамической нагрузки, отражением которой служит суточный профиль артериального давления, а также от активности симпатоадреналовой и ренин-ангиотензиновой системы, уровня гиперинсулинемии. Процессы гипертрофии тесно связаны с активацией синтеза соединительной ткани, о чем свидетельствует повышение уровня трансформирующего ростового фактора-Pj и N-терминального проколлагена III типа в сыворотке крови. На синтез соединительной ткани существенное влияние оказывает содержание альдостерона, инсулина.
2. Фибробласты, благодаря контактному взаимодействию и изменению структуры внеклеточного матрикса, влияют на процессы гипертрофии кардиомиоцитов, что открывает новые возможности контроля за гипертрофическим процессом через изменение функционального состояния клеток стромы. Ингибиторы гидрокси-метил-глютарил коэнзим, А редуктазы в терапевтических концентрациях подавляют процессы пролиферации сердечных фибробластов, их фенотипическую трансформацию и предупреждают развитие гипертрофии кардиомиоцитов.
3. Повышение уровня фактора Виллебранда, сосудистого ростового фактора в плазме крови и концентрации белка теплового шока 70кДа в сыворотке крови служит маркером повреждения органов-мишеней при гипертонической болезни.
4. Структурные изменения ретинальных артерий тесно связаны с нарушением эндотелийзависимой вазодилатации в лучевой артерии, что подтверждает системный характер эндотелиальной дисфункции при гипертонической болезни.
5. Сдвиг метаболизма оксида азота в сторону образования пероксинитритов и S-нитрозотиолов в условиях оксидативного стресса и дефицита антиоксидантной защиты может служить дополнительным фактором, потенцирующим развитие вазоконстрикторных реакций при гипертонической болезни.
6. Нарушение эндотелийзависимой вазодилатации в пробе с реактивной гиперемией возникает на фоне морфологических изменений в интиме лучевой артерии. Гиперплазия интимы лучевой артерии зависит от степени и давности артериальной гипертензии.
7. Медикаментозная коррекция интимальной гиперплазии возможна при использовании комплексной терапии, включающей ингибиторы ангиотензин-превращающего фермента, бета-адреноблокаторы и ингибиторы гидрокси-метил-глютарил коэнзим, А редуктазы.
8. Нарушение реологических свойств крови влияет на процессы ремоделирования миокарда и сосудов у больных ГБ. Действие реологических факторов реализуется через изменение функционального состояния эндотелия и увеличение продукции трансформирующего ростового фактора-Pi.
9. У больных гипертонической болезнью выявлено повышение функциональной активности лейкоцитов, а также уровня межклеточных адгезионных молекул ICAM-1, что свидетельствует о развитии провоспалительных условий. Активация лейкоцитов при гипертонической болезни служит дополнительным фактором повреждения эндотелия и нарушения его функции.
10.Регресс гипертрофии левого желудочка у больных гипертонической болезнью зависит от степени снижения артериального давления и влияния антигипертензивной терапии на негемодинамические факторы ремоделирования миокарда. Длительная адекватная гипотензивная терапия эналаприлом и нифедипином прологированного действия, благодаря позитивному влиянию на функциональные свойства клеток крови и эндотелиоцитов, способствует обратному ремоделированию структурных изменений миокарда в отличие от терапии атенололом. 11. Снижение интенсивности синтеза коллагена III типа на фоне терапии эналаприлом свидетельствует о позитивном влиянии препарата на процессы сердечного фиброза у больных гипертонической болезнью, но уменьшение продукции сосудистого ростового фактора может оказывать негативный эффект на процессы ангиогенеза.
ПРАКТИЧЕСКИЕ РЕКОМЕНДАЦИИ Ингибиторы АПФ и блокаторы кальциевых каналов дигидропиридинового ряда длительного действия при достижении целевого уровня АД обладают наибольшим модулирующим эффектом на негемодинамические факторы ремоделирования миокарда и сосудов, способствуя регрессу структурных изменений.
Методы операционного забора и сохранения лучевой артерии, а также протокол преди послеоперационного ведения больных ИБС должен строиться с учетом индивидуальных факторов риска и морфофункциональных особенностей используемых для аортокоронарного шунтирования артерий. В рамках предоперационной подготовки больного для аортокоронарного шунтирования целесообразно назначение комплексной лекарственной терапии, включающей бета-адреноблокаторы, ингибиторы АПФ и ингибиторы гидрокси-метил-глютарил коэнзим, А редуктазы.
