Фармакологическая профилактика внезапной сердечной смерти у больных с постинфарктной дисфункцией левого желудочка (реферат, курсовая, диплом, контрольная)
Актуальность исследования. В первый год после перенесенного инфаркта миокарда (ИМ) смертность больных особенно высока, причем до
50% пациентов умирает внезапно в результате желудочковой тахиаритмии (В.А.Сулимов, 2004). Наибольший риск внезапной сердечной смерти (ВСС) имеют пациенты с фракцией выброса левого желудочка (ЛЖ) менее 40%. Согласно современным международным Рекомендациям по лечению желудочковых аритмий у всех таких больных рассматривается возможность применения имплантируемого кардиовертера-дефибриллятора (ИКД) с целью первичной профилактики ВСС (D.Zipes et al., 2006). Однако, учитывая крайне высокую стоимость ИКД, эти аппараты реально используются у ничтожно малой части из тех пациентов, которым они показаны (H.Ector, P. Vardas, 2007).
По данным крупных рандомизированных исследований ИКД у больных с постинфарктной дисфункцией ЛЖ эти аппараты за 4-летний период лишь в 40% случаев активно купировали желудочковые тахиаритмии, чаще оставаясь пассивными мониторами электрокардиограммы (ЭКГ) (G.Boriani et al., 2007). Поэтому точная идентификация пациентов с максимальным риском ВСС является остро актуальной (W.Jung, B. Schumacher, 2007). При этом очевидно, что предпочтительны методы исследования, отличающиеся простотой, доступностью и низкой стоимостью.
К сожалению, ИКД может предотвратить ВСС лишь на финальном этапе развития событий и не предупреждает рецидив желудочковой тахиаритмии. Напротив, фармакотерапия с доказанным кардиопротективным, антиремоделирующим действием, способностью улучшать прогноз при хронической сердечной недостаточности (ХСН) позволяет предупреждать прогрессию заболевания к ВСС на самых ранних этапах (S.Priori, D. Zipes, 2006). Ингибиторы ангиотензинпревращающего фермента (АПФ), бета-адреноблокаторы, амиодарон и их комбинации могут оказаться полезными в такой ситуации. Рандомизированные проспективные исследования с целью определения оптимальной тактики фармакологической профилактики ВСС у больных с постинфарктной дисфункцией ЛЖ представляются актуальными.
Цель исследования — разработать способ прогнозирования и фармакологического предупреждения ВСС у больных, перенесших ИМ. Задачи исследования.
1. Оценить прогностическое значение барорефлекторной чувствительности у больных, перенесших ИМ.
2. Сравнить влияние периндоприла, его комбинаций с бета-адреноблокаторами и амиодароном на барорефлекторную чувствительность и прогноз пациентов с постинфарктной дисфункцией ЛЖ.
3. Сопоставить антиремоделирующее действие периндоприла, его комбинаций с бета-адреноблокаторами и амиодароном у больных с постинфарктной дисфункцией ЛЖ.
4. Провести ретроспективную оценку антиаритмической эффективности оригинального амиодарона и его генерических форм.
Научная новизна.
1. Впервые установлены различия скоростей восстановления барорефлекторной чувствительности при лечении периндоприлом, липофильными и гидрофильными бета-адреноблокаторами, амиодароном, ассоциирующиеся с риском ВСС у больных, перенесших ИМ.
2. Впервые показаны различия антиремоделирующего действия фармакотерапии периндоприлом, липофильными и гидрофильными бета-адреноблокаторами у больных с постинфарктной дисфункцией ЛЖ.
3. Впервые в проспективном исследовании продемонстрировано прогностическое преимущество комбинации бета-адреноблокатора с амиодароном перед монотерапией у больных с высоким риском ВСС после ИМ.
Основные положения, выносимые на защиту.
1. Методически правильное определение сниженной барорефлекторной чувствительности помогает идентифицировать пациентов с высоким риском ВСС после перенесенного ИМ.
2. Комбинация амиодарона с карведилолом наиболее эффективно восстанавливает барорефлекторную чувствительность и улучшает выживаемость больных с постинфарктной дисфункцией ЛЖ.
3. Комбинация периндоприла с карведилолом и амиодароном оказывает выраженное антиремоделирующее действие, восстанавливает систолическую и диастолическую функции ЛЖ, улучшая прогноз больных с высоким риском ВСС после ИМ.
Практическая значимость. Предложена и апробирована простая и доступная двухэтапная схема стратификации риска ВСС у больных, перенесших ИМ, включающая эхокардиографию и фенилэфриновый тест. Представлено обоснование целесообразности применения периндоприла, липофильных бета-адреноблокаторов и амиодарона у пациентов с сочетанием постинфарктной дисфункции ЛЖ и снижения барорефлекторной чувствительности. Примененная комбинированная терапия ограничивает постинфарктное ремоделирование ЛЖ, улучшает его функцию, снижает риск ВСС и смерти от прогрессирования ХСН. Показано, что для максимальной реализации антиаритмического потенциала амиодарона следует применять оригинальный его препарат.
Научная значимость исследования. Показано прогностическое превосходство фармакотерапии, ингибирующей ренин-ангиотензин-альдостероновую и симпатико-адреналовую системы, оказывающей выраженный органопротективный и антиаритмический эффект у больных, перенесших ИМ.
Апробация результатов исследования. По теме диссертации опубликовано 16 научных работ, в том числе 2 статьи в журналах, входящих в Перечень ведущих рецензируемых журналов и изданий, рекомендованных ВАК Министерства образования и науки России. Материалы и основные положения работы представлены и обсуждены на Российских национальных конгрессах кардиологов (Москва, 2001, 2003; С.-Петербург, 2002), II Всероссийской конференции по профилактической кардиологии (Саратов, 2002), II съезде кардиологов Южного федерального округа (Ростов-на-Дону, 2002), Международных конгрессах по паллиативной медицине и реабилитации (Анталья, Турция, 2002, 2003), Конференции терапевтов Южного федерального округа (Ростов-на-Дону, 2003), Конгрессе кардиологов стран СНГ (С.-Петербург, 2003), IV Международном конгрессе по электростимуляции и электрофизиологии сердца (С.-Петербург, 2004), Международном кардиологическом симпозиуме (С.-Петербург, 2006). Диссертация апробирована на совместном заседании кафедры госпитальной терапии и кафедры факультетской терапии Кубанского государственного медицинского университета 26 сентября 2008 г.
Объем и структура работы. Диссертация изложена на 132 страницах машинописного текста, состоит из введения, обзора литературы, описания материала и методов исследования, трех глав результатов собственных исследований, обсуждения, выводов, приложения. Библиографический указатель содержит 21 источник на русском и 321 — на иностранных языках. Работа иллюстрирована 11 таблицами и 4 рисунками.
Выводы
1. Фенилэфриновый тест позволяет адекватно оценивать барорефлекторную чувствительность, определяя риск внезапной сердечной смерти, только при исключении действия на обследуемого факторов, способствующих активации симпатико-адреналовой системы.
2. Периндоприл восстанавливает барорефлекторную чувствительность у больных с постинфарктной дисфункцией левого желудочка и его действие усиливается амиодароном, метопрололом, бисопрололом, карведилолом, но не атенололом.
3. Комбинация амиодарона с карведилолом обеспечивает наибольшее увеличение исходно сниженного показателя барорефлекторной чувствительности и снижение общей смертности больных с постинфарктной дисфункцией левого желудочка как за счет случаев внезапной сердечной смерти, так и прогрессирования сердечной недостаточности.
4. В течение года лечения пациентов с постинфарктной дисфункцией левого желудочка антиремоделирующий эффект периндоприла усиливают карведилол и метопролол, но не амиодарон, бисопролол и атенолол. Липофильные бета-адреноблокаторы повышают фракцию выброса левого желудочка, но его диастолическую функцию улучшает только карведилол. Комбинация периндоприла с карведилолом и амиодароном наиболее быстро корригирует нарушения геометрии и функции левого желудочка.
5. Лечение оригинальным препаратом амиодарона, по сравнению с его генерическими формами, позволяет добиваться большего антиаритмического эффекта и уменьшать риск возможных осложнений терапии в реальной клинической практике.
Практические рекомендации
1. Пациентам, перенесшим инфаркт миокарда, с фракцией выброса левого желудочка ниже 40% по данным эхокардиографии целесообразно выполнение фенилэфринового теста в условиях, исключающих влияние факторов, способных исказить его результаты. Для этого при определении показателя барорефлекторной чувствительности следует пунктировать вену за 15−20 минут до введения фенилэфрина, проводя в течение этого времени медленную капельную инфузию 50 мл 0,9% раствора хлорида натрия. Исследование необходимо проводить в привычной для пациента обстановке его палаты, без перевода в манипуляционную, процедурный кабинет и т. п.
2. Для успешного восстановления сниженной барорефлекторной чувствительности у больных с постинфарктной дисфункцией левого желудочка оптимально подходит комбинация периндоприла с липофильным бета-адреноблокатором и амиодароном.
3. С целью коррекции постинфарктного ремоделирования левого желудочка, первичной профилактики внезапной сердечной смерти и снижения риска фатального исхода от прогрессирования сердечной недостаточности целесообразно назначать комбинацию периндоприла с карведилолом и амиодароном.
